Three NDIS templates we wish we'd had ten years ago.
The incident report, shift notes and support plan templates we use inside Aura OS — released free, branded to your operation, ready to drop into Word, Google Docs or print.
No upsell. No paywall. No "enter your credit card to access the free download." Drop your email, get the template. We'll send one short email a week for the next month with a useful thing each time — unsubscribe any time, no awkwardness.
Built alongside SIL operators in conversation every day.
Incident Report Template
Five sections covering the who, what, where, when and what next. Built for the NDIS Commission reporting flow — drop it into your incident process tomorrow without modification.
- Reportable + non-reportable categorisation included
- Witness statement page included
- Investigation step prompts on page 3
Shift Notes Template
A one-page shift notes template that holds up under audit. Time in, time out, observations, actions taken, handover ack — the structure auditors look for, in plain language.
- Wellbeing gate check on shift start
- Handover acknowledgement field
- Behaviour-support observation prompts
Support Plan Template
A participant-centred support plan template. Goals, supports, daily routines, behaviour-support strategies, escalation path. Reviewable, signable, branded with your logo once you add it.
- Goals + outcomes section
- Behaviour-support summary
- Review-date prompts every 6 months
The first thing an operator does is fix the templates.
Every NDIS operator I know has a folder somewhere with a half-finished incident template, a shift notes form somebody copied off a previous job, and a support plan in Word that's never been updated. They mostly work. They mostly don't get rewritten. They are absolutely the documents an auditor reads first.
We built these as Clearline-branded versions of the templates that come pre-loaded inside Aura OS. Same structure. Same audit-ready logic. Released here for anyone running a small operation who isn't (yet) on Aura OS — because the templates aren't the moat. The platform is.
Use them as-is. Customise them to your operation. Roll them into your existing system. They're yours.
Built alongside operators, not by consultants.
Six questions before you download.
Are these really free?
Can I customise these to my operation?
What's the catch?
Will I get spammed?
Are these aligned with the 2026 NDIS Practice Standards?
I want all three — can I download them in one go?
Free templates fix today. Aura OS fixes the next twelve months.
Templates work great for two houses and one shift a day. But once you're running three houses, eight workers, twenty participants and a Commission audit in the diary — you need the system, not the document. Aura OS is the system. Free for your first two participants, then a simple price by participant as you grow, never per-worker. No card.
See how Aura OS works →Or just keep the templates. No hard feelings. We'll see you in a year when your operation has grown.
