Free for any NDIS operator

Three NDIS templates we wish we'd had ten years ago.

The incident report, shift notes and support plan templates we use inside Aura OS — released free, branded to your operation, ready to drop into Word, Google Docs or print.

No upsell. No paywall. No "enter your credit card to access the free download." Drop your email, get the template. We'll send one short email a week for the next month with a useful thing each time — unsubscribe any time, no awkwardness.

Built alongside SIL operators in conversation every day.

Why we built these

The first thing an operator does is fix the templates.

Every NDIS operator I know has a folder somewhere with a half-finished incident template, a shift notes form somebody copied off a previous job, and a support plan in Word that's never been updated. They mostly work. They mostly don't get rewritten. They are absolutely the documents an auditor reads first.

We built these as Clearline-branded versions of the templates that come pre-loaded inside Aura OS. Same structure. Same audit-ready logic. Released here for anyone running a small operation who isn't (yet) on Aura OS — because the templates aren't the moat. The platform is.

Use them as-is. Customise them to your operation. Roll them into your existing system. They're yours.

Built alongside operators, not by consultants.

The obvious questions

Six questions before you download.

Are these really free?
Yes. No payment, no card, no trial that auto-converts. Drop your email, get the template. Unsubscribe any time and the templates are still yours to keep.
Can I customise these to my operation?
Yes. We send PDFs with editable Word and Google Docs versions linked from the back page. Rebrand them, change the wording, add your logo — they're yours.
What's the catch?
No catch. We make Aura OS, a $49-a-month NDIS compliance platform. If these templates are useful to you, the full platform probably is too. But the templates are not a back-door funnel. We don't lock them, we don't upsell on them, and the only mention of Aura OS in the template PDF is on the back cover.
Will I get spammed?
One email a week for four weeks. Each one has one specific, useful thing — usually a piece of compliance content built around what's in the template. After four weeks, you're moved to our monthly digest (which you can unsubscribe from any time).
Are these aligned with the 2026 NDIS Practice Standards?
Yes. The incident report and support plan are mapped to the relevant Practice Standards modules. The shift notes template is structured to feed evidence directly into the six-pillar audit framework we use inside Aura OS.
I want all three — can I download them in one go?
Yes — drop your email on any one of the three cards and we'll send all three to the same inbox. Submit on the first card, then close the others.
When a template stops being enough

Free templates fix today. Aura OS fixes the next twelve months.

Templates work great for two houses and one shift a day. But once you're running three houses, eight workers, twenty participants and a Commission audit in the diary — you need the system, not the document. Aura OS is the system: $49 a month, flat — a flat monthly price with no per-user tax. Free 7-day trial, no card.

Or just keep the templates. No hard feelings. We'll see you in a year when your operation has grown.
